Commitment to Treatment
We believe in educating you about your dental health and helping you choose treatment options that are best for you. Once you have selected a treatment option and that treatment has begun, we ask that you make every effort to see the process through to completion.

Commitment to Appointment
Because we respect the importance of your time, we work very hard to schedule appointments convenient for you. In return, we ask that you respect us by arriving at your scheduled time. If you find it necessary to change an appointment, please give at least 48 hours advance notice. Short notice cancellations and broken appointments mean lost time and are a disappointment for us and those patients who could have used that time. A charge of $50.00 may be applied to your account for failing to honor a reserved appointment. We ask that you mark your appointments on your calendar as firm commitments to us and to your health.
Commitment to Financial Arrangement
You will be fully informed of fees for services and, if you need clarification, we will be happy to review them with you prior to treatment. While we do submit insurance claims, we ask that you be prepared to pay your estimated portion at the time of service. Since it is only an estimate, you are responsible for the balance remaining after insurance payment is received. Please see our "Insurance Benefits Verification" form..
- New patients—cash, check, debit or credit card at the first visit.
- Treatment under $300—cash, check, debit or credit card at the time of service.
- Treatment over $300—arranged with the business manager.
